What is Support Coordination?

Atria Health Care will work with you to get to know you and to build your capacity to understand the NDIS, whilst ensuring you are getting the most out of your approved plan, and that your goals are being achieved.

We aim to build your ability and confidence to manage and coordinate your services. By working together with our Support Coordinator serving throughout Australia, you can:

  • Understand your plan and the supports and services you would like to access.
  • Connect with quality NDIS providers that are right for you.
  • Coordinate assistance from mainstream services such as education, health and housing.
  • Work through challenges and resolve issues.
  • Feel more confident in managing your plan independently in the future.
When you start with Atria Health Care, your support coordinator will check in with you and your service providers regularly to make sure things are working well. Reports are produced during key milestones, and a meeting is organised near the end of your plan to talk about your future goals and needs.

Atria Health Care also provides Level 3 Specialist Support Coordination and Recovery Coach.

Who can access these services?

Support Coordination is funded by the National Disability Insurance Agency (NDIA) and is allocated to assist you with implementing your plan.
